No. It plugs into a standard 120 V household outlet. Use an outlet on its own circuit if you can, and don't run it on an extension cord or power strip.
Yes. The flame and the heater have separate controls, so you can keep the fire on all year with the heat off.
The heater runs at 750 W or 1,500 W and takes the chill off a room of about 400 sq ft. It's zone heat for the room you're in. It won't replace your furnace.

Went with the 65" under our 65" TV. The trim is thin and sits tight to the drywall, so it looks like the builder put it there. We tried the blue flame once for fun. It lives on orange.
I was worried the heater would be loud. On high you hear a soft fan, about like a bathroom fan, and with the heat off there's nothing. The remote is simple enough that my 7-year-old runs it.
Take the cut-out size seriously. I framed to the spec sheet and had about an eighth of an inch to spare on each side. The box is huge and you need two people to lift it in, but nothing arrived damaged.
